What is Thumb Arthritis?

Thumb arthritis, also known as basal joint arthritis, occurs when the cartilage in the thumb carpometacarpal (CMC) joint wears down. This joint, located at the base of the thumb, enables a wide range of motion, including gripping, pinching, and rotation.

When this cartilage wears away, the bones of the thumb joint begin to rub against each other, causing pain, inflammation, and reduced motion. The CMC joint is particularly susceptible due to its high usage in daily tasks and the stress placed on it when you grip things or pinch.

Symptoms of Basal Thumb Arthritis

Basal thumb arthritis symptoms usually develop gradually and worsen over time. You may notice:

  • Pain and tenderness at the base of the thumb
  • Swelling, stiffness, and inflammation
  • A grinding or clicking feeling when moving the thumb
  • Difficulty pinching, grasping, or turning objects like keys
  • Reduced strength and the inability to glide easily during motion
  • Discomfort that worsens in daily living or after overuse
 

These symptoms can make it hard to perform simple tasks like opening jars or using your smartphone.

Causes and Risk Factors

The most common cause of thumb arthritis is osteoarthritis, a degenerative condition where joint cartilage wears away over time. Several factors may increase your risk, including:

  • Age (more common over 40)
  • Female gender
  • Prior thumb injuries
  • Joint laxity or instability
  • Repetitive stress on the thumb joint
  • Genetics
 

The condition may also coexist with arthritis in other joints, especially in the hands and wrists.

Diagnosis

Diagnosis typically involves a physical examination and imaging tests. Your doctor may:

  • Examine the joint at the base of your thumb for tenderness, swelling, and deformity
  • Move the thumb joint to assess pain and motion
  • Order X-rays to evaluate joint space, bone damage, and alignment of the metacarpal bone and trapezium

Treatment Options

Thankfully, basal thumb arthritis is a very treatable condition. Treatment plans often begin conservatively with options for surgery if symptoms worsen:

Non-Surgical Treatments

  • Rest and activity modification
  • Use of thumb splints or braces
  • Over-the-counter or prescription pain relievers
  • Corticosteroid injections to reduce inflammation
  • Hyaluronic acid injections to lubricate the joint
  • Occupational therapy to strengthen the thumb and improve function
 

These measures often provide relief for several months or longer.

Surgical Options

If conservative treatments don’t provide enough relief or if you’re struggling to bend, twist, or use your thumb effectively, your doctor may recommend surgery to restore function and reduce pain. Several surgical procedures are available, depending on the severity of your condition and your daily activity needs:

  • Joint fusion (arthrodesis): This procedure permanently fuses the bones in the affected thumb joint, creating a stable structure that can bear weight without pain. However, the fused joint will no longer have flexibility.
  • Osteotomy: In this surgery, the bones of the thumb carpometacarpal (CMC) joint are repositioned to improve alignment and correct deformities, which can reduce discomfort and improve function.
  • Trapeziectomy: This involves removing one of the small bones at the base of the thumb to relieve pressure and pain in the joint.
  • Joint replacement (arthroplasty): In this approach, all or part of the damaged joint is removed and replaced using a graft, often taken from a tendon in your forearm, to cushion the space and restore movement.
 

These procedures can dramatically improve the quality of life for many patients, particularly when thumb arthritis has progressed to the point of significantly limiting motion or causing constant thumb pain.
