What is Orthobiologics for the Knee?

Orthobiologics is a new and potentially groundbreaking treatment for knee pain that uses natural cells, tissues, and proteins to stimulate the body’s healing process and reduce inflammation in the affected area. 

Orthobiologics involve injections of proteins, enzymes, and/or cells into the area around the injured site. These bioactive materials stimulate a healing response in the body to help promote long-term pain relief, improved function, and better overall health.

What Are the Different Types of Orthobiologic Treatments?

Orthobiologic treatments are a group of minimally invasive treatments that involve the use of natural materials or man-made substances to help promote healing in the body. Some of the most common orthobiologic treatments are listed below.

Autologous Growth Factor Therapies

This involves harvesting the patient’s own blood and processing it to extract components such as platelets and white blood cells that have powerful healing properties. This solution is then injected into the affected area, which works by stimulating repair processes at the injury site.

Platelet-Rich Plasma (PRP) Therapy

This treatment involves harvesting the patient’s own platelets from a sample of their blood. The platelets are then processed and injected into the affected area. These PRP injections release growth factors to help facilitate healing.

Stem Cell Therapy

Stem cell therapy is a form of regenerative medicine that involves injecting stem cells or progenitor cells into an injured area in order to potentially promote the regeneration of damaged tissue. This type of treatment has shown promising results for conditions such as arthritic joint pain, rotator cuff tears, and ligament injuries. Most commonly, stem cells are obtained from bone marrow in your pelvis.

Bone Marrow Aspirate Concentrate (BMAC)

This treatment involves processing marrow that is removed from your pelvic bone. This marrow produces specialized stem cells that produce the proteins necessary for healing and inflammation.


Prolotherapy is a technique used to stimulate healing by injecting an irritant solution directly into the injured area. The solution causes inflammation which stimulates the body to respond with a healing process, including the production of new collagen tissue.

Hyaluronic Acid (HA) Injections

This is a naturally-occurring substance found in the body’s connective tissues and joints. When injected into the knee joint, it serves to cushion and lubricate the joint, restoring normal movement and reducing pain.

Hyaluronic acid injections can provide short-term relief from symptoms such as pain, swelling, and reduced mobility—allowing for improved function after a period of rest or rehabilitation. Additionally, hyaluronic acid has been shown to reduce inflammation within the joint capsule—decreasing long-term damage to tissues such as cartilage and bone. 

Physical Therapy

The goal of physical therapy in orthobiologic treatment is to reduce pain, promote healing, restore movement, and ultimately enable patients to return to their desired activities quickly and safely.

This may include manual therapies such as joint mobilization, massage, and soft tissue manipulation; therapeutic exercises; modalities like heat or cold application; electrical stimulation; water therapy; biofeedback; balance training; gait training; home exercise programs; and patient education.

When combined with orthobiologic treatments, such as platelet-rich plasma (PRP) and stem cell injections, physical therapy can assist the body’s natural healing process and help patients reach their desired outcomes more quickly.
