What is De Quervain's Tenosynovitis?

De Quervain’s tenosynovitis (also known as De Quervain’s disease) affects the tendons that run along the thumb side of your wrist. These tendons pass through a sheath that keeps them close to the bone. These tendons can become constricted if the tendons swell or the sheath thickens. This can cause friction or pain through certain thumb and wrist movements.

You may get De Quervain’s Tenosynovitis because of the following:

  • Repetitive overuse of the thumb and wrist
  • If you’re pregnant or breastfeeding
  • You may be more susceptible if you have rheumatoid arthritis
  • If you’re in your 40s or 50s
  • Women are more affected than men

About De Quervain's Release Surgery

De Quervain’s release surgery is a surgical procedure used to treat De Quervain’s tenosynovitis. Your surgeon will release (cut) the tendon sheath to allow more room for the tendons to glide freely. The procedure will relieve friction and pain without affecting the function of your hand/wrist.

Candidates for the Surgical Procedure

You could be a candidate for De Quervain’s release surgery if you have severe symptoms or if non-operative treatments, such as a steroid injection, have not been effective.

De Quervain’s tenosynovitis can usually be treated using non-surgical treatments such as splints, NSAIDs, and steroid injections.

How to Prepare For De Quervain's Release Surgery

For 6 to 12 hours before De Quervain’s release surgery, you will need to stop eating or drinking. You will also need to arrange for someone to drive you home after the procedure.

You can improve recovery and reduce complications by taking the following steps:

  • Stop smoking
  • Maintain a healthy weight
  • Regularly exercise

The De Quervain's Release Surgery Procedure

De Quervain’s release surgery is an outpatient procedure, meaning you can return home the same day after surgery. This is often performed in the office with local anesthesia. 

The surgical procedure will usually follow these steps:

  1. Your surgeon will first locate and protect the nerves close to the tendon sheath.
  2. A small incision is then made near the base of your thumb.
  3. The tendon sheath is released to allow more space while preserving tendon stability.
  4. Any excess tissue will be removed from around the tendon.
  5. The surgeon will then close the incision and bandage the wound.
  6. In some cases, a splint will also be used.

Potential Complications of De Quervain's Release Surgery

There is a potential to experience some general complications with the surgical procedure. These include pain at the surgical site, bleeding, scarring, infection, or an allergic reaction. 

The Recovery Process

Your hand should be bandaged and kept elevated for at least 2 days. You will be encouraged to gently exercise your fingers, elbow, and shoulder to avoid stiffness. You may required to use the splint for 1-4 weeks. You can expect full recovery anywhere between 6 and 12 weeks.
